Health Assistance Application

The trustee for the Kokatha Charitable Trust is Kokatha Holdings Pty Ltd (“the Trustee’).
The Kokatha Charitable Trust distributes a percentage of funds earned by the Kokatha People Native Title Compensation Charitable Trust. The Kokatha Charitable Trust exists to meet the charitable needs of Kokatha individuals and families.

All Kokatha people are eligible to make application for assistance with Health needs as detailed on the following page.

Health applications can be submitted at any time throughout the year.

Assistance is limited to $2,000 per person per year (July to June)

More than one application can be submitted throughout the year until the annual limit has been reached.

Applications will not be approved if alternative avenues are available, including other Trusts and Government grants.

Useful Hints:
• Ensure you have a complete application.
• Ensure you have a full and complete quote by an approved provider/supplier. The quote or invoice should be issued in your name NOT in the name of Kokatha Charitable Trust.
• If you are applying on behalf of a child, a child in your care, or an Elder, please provide appropriate evidence, such as Medicare Card, Pension Card, or a Centrelink Letter.
• Once an application is lodged with the Kokatha office there is no capacity to alter or make changes to your application.
• All applicants will be notified by phone or email once the application has been considered.
• Once approved the processing and payment of applications will be done as soon as possible.

Note: Funds will not be provided to replace any government benefit or funding program. Cash payments and reimbursements to applicants will not be made.

The aim of this program is to provide support for Kokatha individuals and their Kokatha children needing assistance with expenses to improve and/or manage their Health.

Assistance is limited to $2,000 per person per year, including children, subject to available funds.

Items that can be applied for include:
Dental
Optical
Hearing Aids
Podiatry
Chiropractic
Physiotherapy
Speech Therapy
Counselling
Health Insurance
and other medical treatments recommended by a Medical Practitioner.

Preventative Health measures for disease prevention (such as vaccinations, quit smoking programs) can also be supported at the discretion of the Trustee

All applications must include a letter from a Medical Practitioner, or other supporting documentation including quotes for treatment.

Note:The Trustees discretion can be used for other funding in regard to people with disabilities.

Decisions: This document shall be used as a guide by the Trustee in relation to distribution from the Trust. The Trustee decision in relation to any distribution from the Trust shall be at the absolute discretion of the Trustee. The Trustee has no obligation to provide any reasons for any decision it makes in relation to distribution but at all times will endeavour to act in a sensitive manner within the confines of its legal obligations to administer the Trust appropriately.
